Finance and insurance

Capernaum Medical Center is dedicated to providing our patients with care that is both effective and affordable. Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. We offer a variety of payment options to meet your needs. We accept American Express, Visa, MasterCard and Discover credit cards for your convenience.

If you have any questions about financing or payment, please call us.  We will thoroughly explain your choices and work hard to accommodate you. Remember we are here to help!

Insurances and Referrals

We accept all PPO’s, most HMO’s, Medicaid and Medicaid third parties. Including Aetna (Commercial), Amerigroup, Avmed, Blue Cross Blue Shield (Fl Blue), Cigna, CMS (Med3000), Magellan, Medicaid, Molina, Multiplan, Prestige, Stay well (WellCare), Sunshine, Tricare, United Healhty Care (Commercial). We currently DO NOT accept the following insurances: Aetna Better health, Better Health, Coventry.

Please contact your insurance provider to determine requirements for referral prior to your visit. HMO plans require a referral from your child’s primary care provider prior to your visit. Without proper documentation, your appointment may be cancelled or rescheduled.

Please bring your insurance card to your appointment. Please come to your appointment prepared to pay your co-pay, deductibles, co-insurance and/or any other patient responsibility amounts. These are due at the time services are rendered. We accept cash, checks and credit cards.

Medical Records

Please allow up to 30 days for medical record requests to be completed. The cost of medical records is a flat fee of $6.50 for up to 30 pages or 0.11 cents per page for records over 30 pages, in accordance with Florida State law. Records are faxed free of charge to pediatricians and other specialists caring for the patient. Records can also be viewed and downloaded through the patient portal, but content may be limited.
